PURPOSE: To present the level of knowledge, attitude, and practice (KAP) for glaucoma management and their determinants among adult Saudi patients. METHODS: The study was conducted between January and June 2017. Glaucoma patients from the ophthalmology clinic were interviewed. Demographics and disease-related information were noted. The questionnaire included four K questions, seven A questions, and six P squestions. The correct response of questions were summed for K, A, and P, and the percentile was calculated. K and P scores were graded as excellent (more than >75%), good (51%–75%), poor (26%–50%), and very poor (≤25%). If A score was >50%, it was termed as positive. RESULTS: We interviewed 263 adult glaucoma patients. Their mean age was 61.6 ± 14.2 years. The excellent grade of knowledge about glaucoma and its management was 63 (24% [95% confidence interval (CI) 18.8–29.1]). Poor grade of knowledge was 95 (36.1% [95% CI 30.3–41.9]). The attitude toward glaucoma and its management was positive in 89 (33.8% [95% CI 28.1–39.6]). The practice to manage glaucoma was of excellent grade in 10 (3.8% [95% CI 1.5-6.1]). The overall excellent grade of KAP glaucoma patients was 17.9% (13.2–22.5). Good practice for glaucoma management was associated with a history of glaucoma surgery (P < 0.001), longer duration of glaucoma (P = 0.02), and young age (P = 0.004). CONCLUSIONS: Low KAP about glaucoma disease and medication among Saudi glaucoma patients is a matter of concern. Strategies are needed to improve the KAP of glaucoma patients.

PURPOSE: To evaluate the surgical outcomes in combined hamartoma of retina and retinal pigment epithelium (CHRRPE) using optical coherence tomography (OCT). METHODS: This was a retrospective interventional study, in which medical records of 12 eyes with CHRRPE that underwent vitrectomy and membrane peeling at three tertiary vitreoretinal institutes were reviewed. Preoperative and postoperative color fundus photographs and OCT were reviewed for each follow-up visit. RESULTS: Five out of 12 eyes had a good visual outcome (equal or more than 2 lines gain in visual acuity), four had stable visual outcome (<2 line gain), and three had poor visual outcome (loss of visual acuity). Preretinal fibrosis preoperatively was seen in 80% of cases with good outcomes as compared to 50% and 0% of cases in the cohorts with stable and poor outcomes, respectively. Preoperatively all 5 cases with good visual outcome had Grade 4 epiretinal membrane (ERM), while only 1 case with stable and poor visual outcome had grade 4 ERM and the rest had Grade 3 ERM. Maxi peaks were seen in 80%, 50%, and 0% of cases preoperatively in the three cohorts, respectively. Hyperreflectivity of inner retinal layers preoperatively was evident in all cases having good visual outcomes, in 75% of cases with stable visual outcomes, and in 33% of cases with poor outcomes. CONCLUSION: Surgical intervention in CHRRPE with preretinal fibrosis seems to be beneficial. The visual recovery as defined by conventional predictors in cases of surgical removal of ERM fails to explain the visual outcome in CHRRPE.

PURPOPSE: This study aims to evaluate the ocular surface response after strabismus surgery, using two different materials for conjunctival closure. METHODS: Randomized prospective comparative study was done. After performing strabismus surgery, conjunctival flap was apposed with fibrin sealant in Group 1 and 8-0 vicryl suture with buried knots in Group 2. Preoperative and postoperative measurement of tear film break up time and Schirmer test for tear secretion was done in both the groups of 30 patients each at regular follow-up visit up to 2 months. Postoperatively, both the groups were also compared for the resolution of discomfort and redness. RESULTS: Conjunctival recession was not seen in any group. Mean period was 5.8 days for resolution of discomfort (pain, lacrimation, and irritation) and 7.2 days for redness in Group 1 compared to 12 and 16 days in Group 2 (P < 0.05). During the study, we noticed significant decrease in tear film stability and increase in tear secretion in both the groups. However, in Group 2, the parameters were significantly more altered and even changes persisted for longer duration. CONCLUSION: Early rehabilitation of ocular surface might be related to unaltered healing process with the use of a biological substance (fibrin sealant) as compared to suture.

PURPOSE: To evaluate the level of awareness and knowledge about glaucoma among Jordanians and determine the relationship between glaucoma knowledge and selected variables. METHODS: This was a descriptive and cross-sectional study conducted at a central hospital. Face-to-face interviews were performed to obtain sociodemographic data and information about glaucoma from Jordanian participants attending different outpatient clinics of Jordan University Hospital (JUH). A convenient sample of 488 participants aged 16 years and above were recruited. Consent form was signed by each participant before starting the interview. RESULTS: The mean age of the study population was 45.71 ± 15.44 years, ranging 16 to 89 years. There were 163 (33.4%) females and 325 (66.6%) males. 81.6% of participants had heard of glaucoma. Only 34.2% of participants defined glaucoma correctly. 52.4% of participants had a low level of knowledge about glaucoma. The main source of information was from family members, relatives, and friends (66.6%); however, this source inversely influenced the level of knowledge. CONCLUSIONS: The results of this study indicate a high level of awareness of glaucoma among Jordanians but low-to-average knowledge about it. Health education programs should be activated at all levels of health- and eye-care services to increase knowledge about glaucoma and prevent the irreversible loss of vision due to the second main cause of blindness worldwide.

From the early period of Islam, Muslim scholars have translated the ancient Greek medical works, and they reached a much more advanced level. Blindness was a major cause of disability in all Islamic geography, so physicians from Islamic territory, particularly Yuhanna ibn Masawayh, Hunain Ibn Ishaq, Rhazes, Ali bin Isa, Ibn-i Sina, and Ibn Al-Haitam, were particularly focused on the anatomy of the eye and diagnosis and treatment of eye diseases. In this study, we aimed to report the contributions of the Muslim scholar on the anatomy of the eye and adnexa for the first time in the literature.

PURPOSE: To evaluate the use of non-mydriatic fundus camera as a screening modality for diabetic retinopathy in a sample of population in Riyadh, Saudi Arabia. METHODS: Patients coming, from April 2015 till September 2018, for their diabetic check up at the diabetic center clinics in King Abdul-Aziz University hospital were screened using a non-mydriatic fundus camera (NMFC). Photos were graded by retina specialist and compared to the findings of dilated fundus examination (DFE) by retina specialists. RESULTS: The grading results of NMFC and DFE were compared and the overall sensitivity and specificity for detection of diabetic retinopathy within one grade of retinopathy was 98.7% and 80% respectively. The sensitivity for detection of sight threatening conditions such as proliferative diabetic retinopathy, severe non-proliferative diabetic retinopathy, and diabetic macular edema (by Ocular Coherence Tomography) was found to be 86.7%, 90.3% and 100% respectively; while the specificity was found to be 96.5%, 93%, and 100% respectively. CONCLUSION: Non-mydriatic fundus camera has a high sensitivity and specificity in screening for diabetic retinopathy. It is a great screening tool, which is user friendly and can be operated by trained nurses in primary clinics during patient's regular routine diabetic checkups. It aids in early detection of sight threatening conditions which need urgent referral to ophthalmologists.

Topiramate is a sulfamate-substituted monosaccharide used in the treatment of epilepsy and migraines. This case report demonstrates a 29-year-old female who complained of severe bilateral loss of vision 14 days after using topiramate for her migraine. On initial examination, visual acuities were counting fingers in both eyes. Intraocular pressures were 55 mmHg and 58 mmHg in the right and left eyes, respectively. Anterior segment examination showed bilateral conjunctival chemosis, mild corneal edema, and markedly shallow anterior chambers with closed angles on gonioscopy. Ultrasound biomicroscopy revealed bilateral anterior rotation of ciliary body with choroidal effusion. Topiramate was immediately discontinued, topical and systemic anti-glaucomatous treatment was administered. After 1-week follow-up, the patient had fully recovered.

PURPOSE: Refractive error (RE) is one of the most common ocular disorders among children worldwide. This study aimed to investigate the prevalence of RE and possible risk factors among school children in Riyadh. METHODS: This is a cross-sectional study using data collected at various schools. To achieve the aim of the study, we selected a random sample of 850 school children aged 6–15 years. The examination was based on the RESC protocol and included full visual assessment. Furthermore, a questionnaire was sent to the parents of the participants to ascertain information about lifestyle factors. We constructed a logistic regression model to evaluate the predictors of RE. RESULTS: Close to a third of the children had a RE. Of those identified as having the condition, 60% did not wear glasses (newly diagnosed). Nearly all the children (95.4%) in our sample reported using electronic devices, according to parents. No association was found between using electronic devices and having a RE (P = 0.26). Doing outdoor activities was associated with 52% lower odds of a RE (odds ratio = 1.52). CONCLUSION: We found a higher prevalence of uncorrected RE than previously reported in other Saudi studies. About 60% of children who had RE were not wearing glasses, highlighting the need for a standardized school screening program for early detection and management. Outdoor activities were associated with a lower likelihood of having a RE. These findings might support initiatives to encourage outdoor activities among school children.

PURPOSE: To determine the changes in intraocular pressure (IOP) and anterior chamber parameters following intravitreal bevacizumab injection with different sizes of cannulas. METHODS: This clinical trial was conducted with 70 eyes of 70 patients who received an intravitreal injection of bevacizumab with 26 G (Group 1) or 30 G (Group 2) needle. Preinjection and postinjection 30^th-min IOP and pentacam measurements were obtained. Anterior chamber depth (ACD), anterior chamber volume (ACV), central corneal thickness (CCT), corneal volume (CV), and iridocorneal angle (ICA) measurements were evaluated in pentacam. RESULTS: Preinjection mean IOP values in Group 1 and 2 were 14.7 ± 3.29 mm Hg and 15.1 ± 2.87 mm Hg, respectively. Postinjection mean IOP in Group 1 was 16.8 ± 6.24 mm Hg and in Group 2 was 20.3 ± 3.66 mm Hg. Postinjection mean IOP values were significantly higher than preinjection values in both groups (Group 1 P < 0.005 and Group 2 P < 0.001). IOP change was significantly higher in Group 2 after injection (P < 0.05). In both groups, the change in IOP found to be more significant in phakic eyes than pseudophakic eyes (Group 1 P < 0.001, Group 2 P < 0.001). CCT and CV were significantly higher in both groups 30 min after the injection than preinjection (Group 1 and Group 2; P < 0.01). In Group 2 ACD, ACV and ICA values were significantly lower than preinjection values (P < 0.05). CONCLUSION: Needle bore size is an important parameter that influences anterior segment parameters and IOP change in the intravitreal injection. IOP rise and anterior segment changes are more prominent with a thinner cannula.

PURPOSE: Keratoconus (KC) is bilateral noninflammatory corneal disorder characterized by progressive corneal thinning, protrusion, and scarring. The purpose of this study was to evaluate ganglion cell complex(GCC), macula thickness(MT) and optic head disc parameters in keratoconus patients. METHODS: A hospital based prospective clinical case series was performed in Inonu University School of Medicine. 52 eyes of 52 keratoconus patients and 50 eyes of 50 normal patients were enrolled. RESULTS: There is no statistically significant in MT between groups. GCC in nasal superior, temporal superior and temporal inferior 9 mm from macula were found statistically significant decrease in keratoconus group (p<0,05). In optic disc analysis fifth and the eleventh clock-hour quadrants of peripapiller retina nerve fiber layer and cup area ratio were found statistically significant decrease in keratoconic eyes (p<0,05). CONCLUSION: We thought that structural retinal changes seem in keratoconus eyes; keratoconus pathogenesis may affect not only cornea but also retina and optic nerve head.

We present the case of a 46-year-old man who presented with bilateral panuveitis and occlusive retinal vasculitis 6 months after being acutely admitted with abnormal liver function and diagnosed with primary sclerosing cholangitis (PSC). Initial investigations by the medical and ophthalmic departments including all autoimmune investigations were within normal parameters. Of particular interest was the high likelihood of inadvertent androgenic-anabolic steroid self-suppression of disease. As a lifelong bodybuilder, the patient had been taking oral and intramuscular steroids for years. He became symptomatic upon cessation of these recreational medications. There remains a significant paucity of information describing the relationship between uveitis and PSC. Given the poorly understood aetiology of this rare cholestatic disease, we review the current literature and highlight the diagnostic and therapeutic challenges for such a patient. PSC may predispose patients to an occlusive panuveitis with androgenic-anabolic steroids suppressing ocular autoimmune disease.

PURPOSE: The purpose of this study is to compare the corneal biomechanical properties of hyperthyroids without ophthalmopathy (HWO), thyroid eye disease (TED), and euthyroid participants. METHODS: In this prospective comparative study, one eye per patient included in the analysis and according to exclusion criteria, 38 eyes of 40 HWO patients, 31 eyes of 40 TED patients, and 150 eyes of 160 age- and sex-matched euthyroid participants were enrolled. All participants were evaluated by an endocrinologist and oculoplastic surgeon for evaluation of thyroid function and orbitopathy, respectively. Measurements of corneal biomechanical properties were performed using ocular response analyzer and Corvis ST tonometers. RESULTS: In the HWO group, applanation-1 length (A1 L), applanation-2 velocity (A2V) (P < 0.001), and corneal resistant factor (P = 0.043) were higher than the control group and corneal hysteresis (CH) was lower (P = 0.018). In the TED group, A1 L, A2V (P < 0.001), and highest concavity radius (HCR) (P = 0.027) were higher than the control group, and the CH (P = 0.007) and highest concavity deformation amplitude (HCDA) (P = 0.001) were lower. Furthermore, the level HCDA in the TED group was lower than HWO group (P = 0.011). The level of upper scleral show and palpebral fissure had a negative correlation with CH and HCDA. The amount of exophthalmos level had a positive correlation with HCR. CONCLUSION: Corneal biomechanical properties in the HWO and TED groups were different from the normal euthyroid individuals and may be taken into account in managing situation like glaucoma and refractive surgeries.

Brown-McLean syndrome not reported in association with homocystinuria and generally occurs in patients with long-term aphakia. Presented as peripheral corneal edema with central clear cornea tends to spare vision without the need of any surgical intervention. The purpose of reporting such a rare condition as a first case of Brown-McLean syndrome associated with systemic disease of homocystinuria. Brown-McLean Syndrome in a patient with homocystinuria generally occurs in patients with long-term aphakia after different modalities of surgical intervention. A high clinical suspicion and regular follow-up is warranted for patients with systemic diseases who are aphakic or present with a subluxated lens for early intervention and better prognosis if needed.

CHARGE syndrome is a genetic disorder comprising the following clinical features: coloboma, heart defects, choanal atresia, retardation (of growth and development), as well as genitourinary and ear abnormalities. This syndrome is caused by mutations in the CDH7 gene, located on chromosome 8 (8q12). We present two new gene mutations in two patients with CHARGE syndrome, not previously reported in the scientific literature. Both of these patients clearly demonstrate the difference in the clinical expression of this syndrome, with patient 1 having a greater clinical severity compared to patient 2. We conclude that although in the scientific literature to date there is no clear correlation between a patient's genotype and phenotype expression, we can assume from the cases we present that a correlation does in fact exist. Specifically, missense mutations (as in case of patient 2) are associated with milder clinical expression, whereas mutations which result in truncation of the CDH7 protein (as in the case of patient 1 having a nonsense mutation) may be associated with a more severe clinical expression.

A 24-year-old pregnant woman applied with 2 days of sudden vision loss. Funduscopy showed serous retinal detachment along in the right eye and tear was not found. In the left eye, there were two small subretinal fluid accumulationsThe patient was diagnosed with serous retinal detachment due to preeclampsia. Serous retinal detachment of the patient regressed spontaneously within 1 month. All pregnant women with gestational hypertension should undergo retinal examination.

PURPOSE: Cataract extraction and lens implantation is the most common surgical procedure performed worldwide, with surgeons opting for either a topical or a subtenon anesthetic in majority of cases. While complication rates and pain scores have been previously examined in a bid to determine which modality was better, patients who have had both eyes operated upon under different techniques have never previously been asked which method they prefer. We undertook this study to fill in this crucial gap in our knowledge. METHODS: This is a retrospective study of patients who have undergone bilateral cataract surgery with one eye operated on with topical anesthesia and the other eye with subtenon anesthesia. Patients were identified who had had surgery undertaken in the previous 6 months at Singleton Hospital, Swansea, where both eyes had been operated upon using different anesthetic techniques. Telephone interviews with these patients were performed and their pain scores recorded on a scale from 0 (no pain) to 10 (severe pain) for both eyes, with the patients then being asked which the method of anesthetic they preferred. Statistical analysis of pain scores was compared using Wilcoxon rank-sum testing. RESULTS: One hundred and fifty-two patients in total were identified. One hundred and forty-one of the applicable patients participated in the study. Of these, 78 patients received topical anesthesia for their first operation and 63 had subtenon block for their first operation. The mean pain scores reported for surgery with topical anesthesia and subtenon block were 2.30 and 1.38, respectively. The pain data were analyzed using a Wilcoxon signed-rank test, returning a z-score of 4.038. This result therefore suggests that patients experience statistically significantly less pain from cataract surgery when given a subtenon block. Patients expressed a preference for subtenon anesthesia, with 70% preferring subtenon block, 23% preferring topical anesthesia, and 7% having no preference. CONCLUSIONS: Patients report less pain with subtenon anesthesia compared with topical anesthesia in cataract surgery, where both eyes were operated upon with different anesthetic techniques. This is the first study in which patients who have had both anesthetic techniques performed independently for cataract surgery have had their pain scores reported and statistically analyzed.

Supernumerary lacrimal punctum is rare, and very few cases have been reported. Most patients are asymptomatic, but in some excessive tearing could be a symptom. In addition, obstruction of the canaliculi causing canaliculitis has been reported. We describe four cases in detail with their presentations and diagnosis. The first patient presented with a right eye lesion and during examination, the left eye was less than mid dilated compared to that of the right eye. Further examination of the left eye revealed two left lower lid puncta. In another patient who was evaluated for diabetic retinopathy, two left lower lid puncta that shared the same canaliculus were noted. Furthermore, two lower lid puncta were found in a patient who came for cataract follow-up. Finally, in a patient who was following up in the retinal clinic, two lower lid puncta were documented. Probing to the accessory punctum showed that the canaliculus had an immediate horizontal course, unlike the rest of the puncta which showed a vertical then a horizontal course. Furthermore, in the accessory punctum, there was a soft stop, around 4 mm of the inserting probe. In summary, supernumerary lacrimal punctum is rare, and ophthalmologists need to be aware of this abnormality.

PURPOSE: This study aims to understand the approach to prescribing spectacles for children and the interpretation of refractive errors among ophthalmologists/optometrists in Saudi Arabia. METHODS: A cross-sectional survey was conducted between September and November 2018 using a database of online registrants for ophthalmologists and optometrists practicing in Saudi Arabia. The survey included 10 multiple-choice questions designed to elicit information about how ophthalmologists and optometrists deal with commonly encountered case scenarios covering different practical aspects of pediatric refraction. The responses were compared using the proportion of pediatric clientele received in the respondents' practices (Group A: <25%, Group B: 25%) and drawing upon concepts from the published literature. RESULTS: One hundred and three participants, including ophthalmologists, specialists/registrars, and optometrists from across Saudi Arabia, participated in this survey. Approximately 25% were classified as Group A. Large discrepancies were observed between the participants' responses and actual practices based on guideline recommendations. Responses varied significantly between Groups A and B on certain issues pertaining to pseudomyopia (P < 0.001), anisometropia (P = 0.033), and high astigmatism (P = 0.023). CONCLUSION: Practice patterns regarding pediatric spectacle prescription varied among ophthalmologists and optometrists in Saudi Arabia. The approach to managing refractive errors in children was better among ophthalmologists/optometrists servicing a larger pediatric clientele, suggesting the need to increase awareness among all groups of ophthalmologists/optometrists.

Horner's Syndrome (HS) is a disease characterized by miosis, ptosis, and ipsilateral lack of sweating. It can occur with any injury at the level of the ocular sympathetic system neurons from the hypothalamus to the cervical postganglionic fibers. We present here a case of HS that developed after heart surgery in a 9-year-old boy. Ventricular septal defect, aortic and mitral valves repair, and pacemaker implant procedures were noted in his medical records. Preganglionic HS was diagnosed with bilateral unresponsiveness to a 0.1% adrenaline and positive result in the right eye to 0.5% apraclonidine tests. HS is often related to injuries of the brain stem, upper spinal cord, lung apex tumors and lesions, aortic coarctation, cervical lesions, and carotid lesions have been reported. However, it is rare secondary to heart surgery among the pediatric age group.

We describe a case of an anomalous separation of the medial rectus muscle in an 18-month-old female undergoing strabismus eye muscle surgery for partially accommodative esotropia. During surgery and after hooking the medial rectus muscle, it was noted that the width of the muscle tendon was shorter than usual and that the muscle insertion was displaced inferiorly, this prompt further exploration. It was found that the medial rectus muscle had an anomaly were it was seperated into two (superior and inferior) compartments which were 5 mm apart. This finding may further suggest the compartmentalization theory of the horizontal rectus muscles where the muscle is separated into two nonoverlapping superior and inferior zones, each working independently of the other. In this report, we present the case and suggest surgical technique that can be utilized to address this abnormality, along with the postoperative results.

A 70-year-old male presented with orbital masses affecting the muscular cone. His past medical history was notable for diabetes mellitus, ischemic cardiopathy, sleep-apnea syndrome, and multiple serous effusions. The first biopsy specimen of affected orbital tissue revealed fibrohistiocytic infiltration resembling xanthogranuloma or Erdheim–Chester disease (ECD). An ulterior biopsy of affected orbital tissue showed lymphocyte emperipolesis with immunopositivity for CD68 and S100 but negative staining for CD1a marker, strongly suggesting Rosai–Dorfman disease (RDD). Afterward, pericardium and peritoneal effusions resulted in constrictive pericarditis and retroperitoneal fibrosis, respectively. The absence of distinctive clinical features made the diagnosis especially challenging. Attempts to control the disease using corticosteroids, radiation, orbital surgery, and interferon were unsuccessful. Aggressive treatments such as chemotherapy were not considered appropriate due to the general deterioration of our patient. Although the possibility of two concurrent diseases (e.g., systemic ECD and orbital RDD) cannot be discarded, we interpreted the orbital findings as likely due to RDD, and the entire condition of our patient as an extranodal RDD with atypical clinicopathological findings and outcome.
